Alternatively, clients may not conceal the fact that they desire substances, and would seek out substances if they were able, possibly leading staff to prevent or limit the client\u2019s freedom in order to prevent their access to substances \u2014 often resulting in increased acrimony between clients and staff.<\/p>\n<p>As a result, it is important to under- stand what role, if any, substance misuse may have played in the individual&#8217;s problems to ensure that this is addressed if necessary. How this is addressed will depend on the individual&#8217;s attitude towards their substance using and their motivation to change. 4 Environmental factors: as highlighted, the context in which aggression or violence takes place is extremely important and should form a major part of the assessment process.<\/p>\n<p>A good understanding can be gained from examining the circumstances under which previous acts took place for example, were certain places, certain people and certain times of day significant? Circumstances may not be immediately obvious but can be informed by other assessments and discussions with the individual and carers. For example, an individual&#8217;s delusional beliefs can be important in determining the situations where they may feel uncomfortable or start to become aroused.<\/p>\n<p>People with paranoid beliefs about others may become more distressed when in situations involving other people for example, around mealtimes, in the TV room. People who feel that they are controlled by external agents may feel more threatened when faced with controlling situations such as when being given medication or when faced with routines which they don&#8217;t like. Items from the NAS and the WARS can give clues as to the most likely situations when an individual may become provoked or feel angry.<\/p>\n<p>Information can be gathered from case notes and by questioning key staff who witnessed, or were involved in, the aggression or violence. If there appears to be no particular pattern contributing to the occurrence of violence then some detailed prospective observational assessment may be helpful.
