Hatred Emptiness And Hope – Kernberg

Before the end of the session, I was able to point out to the patient that the relationship between a demanding, frus- trated child and an indifferent, distancing mother had been replicated between us, but now with role reversals: I was now in the role of the de- manding child wanting attention, while he was in the role of his indif- ferent, aloof mother. This short summary illustrates the effort to diagnose the predomi- nant internalized object relationship enacted in the transference: the pa- tient’s frustrated and demanding aspect of his infantile self and the corresponding indifferent and rejecting aspect of his mother.
This rela- tionship was enacted and then reenacted with role reversal that would permit the patient, through interpretation of this repetitive activation, to become aware of the importance of that unresolved conflict in his mind and in his present relationship with me. As part of this process, he became aware of and resolved his unconscious identification with both self and object in this activated dyadic relationship. This relationship was an important component of his tendency to abandon his studies and finding them to be an imposition, his resentment of the teachers’ per- ceived lack of interest and concern, and his provocative efforts to obtain a particular professor’s attention that were being undermined by the ir- ritation he was causing.
All these various aspects of the patient’s difficul- ties at school were now represented in the activation of this particular object relationship, which I was able to help the patient realize was in fact a distortion of reality. It gradually became clear to him that the teacher was neither as rejecting nor as demanding as the patient per- ceived and that his own behavior had provoked the professor to react to him in an irritated and sometimes rejecting way. At the same time, the patient was aware that I was interested in him and that, by his coming to the sessions, carrying out free associations, and being open to an exchange with me, a reality of a cooperative rela- tionship was evolving that corresponded to my role, the patient’s role, his interest in getting help and learning in the treatment, and my con- cerned efforts to help him know himself and the nature of his difficulties.
