Similar to most behavioral interventions, CBT often includes between-session home practice of new pain coping skills (e.g., relaxation practice, activity self-monitoring, completion of thought records). No single CBT treatment protocol exists; rather, several treatment manuals are available for guiding clinicians in delivering CBT and its components (Otis, 2007; Thorn, 2017). Multiple systematic and meta-analytic reviews support the efficacy of CBT for reducing pain intensity, activity interference, distress, and disability across a range of chronic pain populations (Ehde et al., 2014).

CBT has also been shown to be beneficial in the treatment of both youth (Palermo, Eccleston, Lewandowski, Williams, & Morley, 2010) and older adults (Molton & Terrill, 2014) with chronic pain, two age groups in which chronic pain is highly prevalent. Mindfulness-Based Interventions Although the practice of mindfulness dates back centuries, mindfulness- based interventions (MBIs), such as mindfulness-based stress reduction and mindfulness-based cognitive therapy (MBCT), have only recently become popular for addressing chronic pain and its associated suffering. Similar to other behavioral therapies, MBIs typically incorporate a variety of therapeutic techniques but have mindfulness meditation at their core.

Mindfulness meditation encourages individuals to disengage from automatic thinking and to focus attention mindfully on an object of awareness (often the breath) with a sense of nonjudgmental awareness and acceptance of any sensory, emotional, or cognitive events (Day, Jensen, Ehde, & Thorn, 2014). Whereas most CBT interventions are rooted heavily in correcting deficits in thinking and behaving, MBIs builds on individual’s strengths and innate ability to focus their attention at will, and to use these to work for them (as a way to mindfully perceive experience in a nonjudgmental, accepting way) rather than against them (as is often the case, with attention becoming “stuck” on pain, leading to hypervigilance and associated disability; Day et al., 2014).

MBIs are efficacious for treating chronic pain. A recent meta-analysis reported that MBIs reduced pain and depressive symptoms while also improving quality of life (Hilton et al., 2017). In addition to the most studied MBI, mindfulness-based stress reduction, another promising MBI for pain is MBCT (Day, 2017). MBCT integrates cognitive therapy techniques into mindfulness-based interventional strategies to form a streamlined approach to training the mind to respond adaptively to pain.

MBCT focuses on enhancing awareness of how one is attending and responding to pain.
